Proof of income from the past 90 days for all family members within your household related by blood or marriage.

Social Security cards for all family members within your household related by blood or marriage.

If receiving food and nutrition benefits or any other government ​assistance, please bring award letter(s) stating the amount.

Proof of High School diploma or GED.
